On Dec. 6, 2016, Life Care’s Eastern Division held a banquet at Garden Plaza of Greenbriar Cove in Ooltewah, Tennessee, honoring one skilled nursing and rehab facility per each of its five regions. The awards recognize facilities for innovative programs in serving their communities.
Life Care Center of New Market was nominated for improving its discharge planning process in 2016. More departments have gotten involved in preparing residents for discharge, from therapy to nursing to social services to the on-site physician. As a result of these changes, the transitions in health care have gone more smoothly for patients, and the center has been able to care for more patients who require therapy after surgery or illness.
“At Life Care Center of New Market, we now plan for discharge at admission for short-term rehabilitation,” said Natalie Fitzwater, business development director at the facility. “In focusing on each patient’s therapy goals and planning to return home, we are also being good stewards of people’s financial resources and encouraging each person to exercise their choice to live independently.”
